Originally designed to facilitate partnerships with providers for community-based residential treatment for homeless Veterans, many Healthcare for Homeless Veterans (HCHV) programs have evolved into central hubs offering a range of housing and services. These programs serve as a vital means for the VA to connect with and assist homeless Veterans, providing them access to VA care.
At the heart of the HCHV program is outreach, aiming to significantly reduce homelessness among Veterans. The primary objective is to identify and engage the most vulnerable individuals who are not currently accessing services, connecting them with treatment and rehabilitative programs.
Additionally, the Contract Residential Treatment program is a component of HCHV that places Veterans with serious mental health diagnoses into high-quality, community-based, supportive housing.
