Being a State-licensed Home Health Agency, currently authorized across eleven (11) counties, their primary focus lies in ensuring the quality of CARE through a collaborative TEAM approach. This methodology entails engaging with the patient and their caregivers, often family members or close acquaintances, to conduct a thorough assessment of the patient’s existing health condition. This assessment involves reviewing their medical history, medications, and treatments, as well as evaluating the home environment. Additionally, they liaise with the patient’s physician(s) for skilled services and formulate a comprehensive plan of care or a home care plan for non-skilled services. Subsequent discussions with Care Team’s Service Coordinators lead to the assignment of qualified and available personnel for the patient’s care needs.

With a combined experience of over 300 years in the home health industry, their management and office staff are deeply experienced and dedicated.
